From the Account Summary page, select the card containing the transaction you wish to dispute.
Important: This process is for card transactions. If you wish to report an issue with a check or electronic payment transaction, visit the Other Fraud / Dispute Forms page.
Click on the “Report a problem” link to access the dispute form.
Use the drop-down menu to select the reason.
Note: New questions appear based on your selected answer.
Use the drop-down menus to select the impacted card and answer any additional questions.
Review the details on the screen, then click on the “Confirm” button.
If you selected ‘I did not make nor authorize this transaction,’ a message displays notifying you that the card selected will be closed.
Click on the “OK” button to submit the claim.
A confirmation message appears. You will also receive a confirmation email from Logix.
Important: This process allows you to report one transaction at a time. If you have multiple transactions to report, you may call the Logix Member Service Center at 800-328-5328 for assistance.
