Card Transaction Dispute

Report a problem with a card transaction, whether you’re disputing a charge or reporting card fraud.

Select the Card

From the Account Summary page, select the card containing the transaction you wish to dispute.

Important: This process is for card transactions. If you wish to report an issue with a check or electronic payment transaction, visit the Other Fraud / Dispute Forms page.

Transaction History

  1. Locate the transaction you wish to dispute in the transaction history.
  2. Click on the arrow next to the transaction to view more options.

Report a Problem

Click on the “Report a problem” link to access the dispute form.

Select Reason

Use the drop-down menu to select the reason.

Note: New questions appear based on your selected answer.

Additional Questions

Use the drop-down menus to select the impacted card and answer any additional questions.

Details

  1. Type all details for this claim in the text field.
  2. Click on the “Continue” button.

Confirm

Review the details on the screen, then click on the “Confirm” button.

Submit Claim

If you selected ‘I did not make nor authorize this transaction,’ a message displays notifying you that the card selected will be closed.

Click on the “OK” button to submit the claim.

Acknowledgement

A confirmation message appears. You will also receive a confirmation email from Logix.

Important: This process allows you to report one transaction at a time. If you have multiple transactions to report, you may call the Logix Member Service Center at 800-328-5328 for assistance.
