Card-Related Dispute

If the claim you are filing is card related (Debit, Credit) and you need to report a problem, please follow these steps:

  1. Sign in to Online Banking.
  2. From the transaction display, click to expand the card transaction in question.
  3. Click on the "Report a problem" link.

Account-Related Dispute

If the claim you are filing is account related, or ACH (routing and account number were used), please follow these steps:

  1. Download the Written Statement of Unauthorized ACH Debit form.
  2. Complete the necessary information and sign the form.
  3. Send the form to Logix using one of the following:
    • Scan and email the form to [email protected]
    • Fax the form to (818) 565-2094
    • Mail the form and any supporting documents (i.e. emails with the company, receipts, etc.) to:

      Attn: Payments Department
      P.O. Box 4070
      Castaic, CA 91310-4070

Once we receive your completed form, Logix will credit your account within 3 business days.
