Other Fraud / Dispute

Report a non-card related problem such as check, ACH, or account related transactions.

Hamburger Menu

From your Online Banking homepage, click on the hamburger menu icon located in the header.

Forms & Disclosures Menu

  1. Click on the “Forms & Disclosures” tab to expand this menu.

  2. Click on “Check Fraud/Dispute Form”.

A new browser tab will open with the LFCU Check Fraud Dispute Form web page.

Download Form

  1. Review the instructions on the page.

  2. Click on the “Download PDF Form” button to view the form in a new browser tab.

Complete & Send Form

  1. Download the form.

  2. Complete all applicable sections of the form.

  3. Send the form using one of the methods provided in the instruction page (email, fax, or mail).

Download Form

  1. Review the instructions on the page.

  2. Click on the “Download PDF Form” button to view the form in a new browser tab.

Complete & Send Form

  1. Download the form.
  2. Complete all applicable sections of the form.
  3. Send the form using one of the methods provided in the instruction page (email, fax, or mail)
